Member Requirements:
As a Member of World Kickboxing Federations Canada Inc., I understand and swear to adhere to the following:
· Membership is annual and valid for the calendar year in which it is paid, running from January 1 to December
31. Membership dues are not pro-rated and must be renewed each year. Dues payments are to be paid by
January 31 – If a Member does not pay membership dues by the deadline date, the Member will no longer be
entitled to any of the benefits or rights of membership and will be considered to have resigned from World
Kickboxing Federations Canada Inc.· Members will support World Kickboxing Federations Canada Inc. and model appropriate behavior at all times.
Members will focus comments or criticism appropriately and avoid public criticism of athletes, coaches, officials,
organizers, volunteers, employees, members of World Kickboxing Federations Canada Inc. while consistently
demonstrating the spirit of sportsmanship, sports leadership and ethical conduct and practices.· Members will read, understand and comply with the rules of World Kickboxing Federations Canada Inc. as
applicable to the sports being participated in and ensure that these rules are adhered to.· Members are encouraged to use the World Kickboxing Federation logo for promotional purposes. Unauthorized
use is prohibited.· A Member may resign from World Kickboxing Federations Canada Inc. by giving notice in writing via email to: wkfcanada@outlook.com
Release & Waiver
In consideration of membership and permission to become involved with World Kickboxing Federation Canada Inc., I hereby release and discharge the World Kickboxing Federation Canada Inc., its affiliates, clubs, coaches, officials, members, agents, officers, and employees from all claims, actions, judgments and executions which the undersigned’s heirs, executors, administrators, or assigns may have, or claim to have, for all personal injuries, known or unknown and injuries to property, real or personal, caused by, or arising out of participation in the sports activity within World Kickboxing Federation Canada Inc.. I, the undersigned, understand that all styles sanctioned are contact sports and thus have inherent risks involved that can result in injury and/or death. I am fully aware of these risks, but waive rights, claims, cause of action etc., as
heretofore, and do hereby assume the risk completely. I, the undersigned understand that the products and services provided to my club/school/gym by the World Kickboxing Federation Canada Inc., are meant for promotional purposes only and are not intended for general application for all individual members of my club/school/gym, and it is solely my responsibility to administer such products and services to those individuals that I have deemed appropriately prepared for engagement in such programs and services. I also have read and understand the above and agree to fulfill the listed requirements. I understand and agree that any violation of any World Kickboxing Federation
Canada Inc. policy or rule by any member of my club/school including but not limited to, an athlete, coach, instructor or recreational member, will result in the suspension or expulsion of my club from the World Kickboxing Federation Canada Inc.. I also understand that if my Club Membership expires due to my failure to renew, that it is my sole responsibility. My club name, as well as any pictures taken, including those of the events, facilities, coaches and/or athletes, may be used for promotional purposes by World Kickboxing Federation Canada Inc. without financial compensation payable to the club, competitor or otherwise. I, the undersigned, have read this Release and Waiver and understand all of the terms and conditions; I execute it voluntarily and with full knowledge of its significances.

Your personal information is required to register you with World Kickboxing Federation Canada Inc.in any capacity, including, without limitation, as a World Kickboxing Federation Canada Inc. registrant, athlete, coach, official or in connection with your affiliation with a member club or school and to administer various services, such as World Kickboxing Federation Canada Inc. events. Your personal information may also be exchanged with World Kickboxing Federation Canada Inc. affiliates which includes local clubs or school or provincial association or section. By submitting this form, you expressly provide your consent to the sharing of your personal information with World Kickboxing Federation Canada Inc. and as described herein for purposes of registration and receipt of national services delivered by World Kickboxing Federation Canada Inc.. We adopt the 10 Fair Information Principles into our privacy program, and employ reasonable measures to protect against unauthorized access, processing, disclosure, alteration, destruction or loss of your personal information.
